1. Resale or Sales Tax Certificate number

2. Copy of your business card 

At this time, files cannot be uploaded with the application. , please send scanned copies of the two required documents to Shopjuniperway@gmail.com with the name of the applicant listed in the subject line of the email.

Incomplete applications will not be accepted. We reserve the right to make all membership determinations at our sole discretion.

Once you are an approved trade member you will be able to send us direct PO’s and we will apply discounts directly.
